Question for psychotic/schizophrenic patients -

What are your top 3 meds which have improved your overall cognition from most to least ?

I’ll start with mine:

  3. Calendar (haven’t figured out what type of scheduling works best for me yet. Certain people carry a journal, some schedule on their phone, I’m starting to think there’s a lot of people that strictly go off of monthly calendars.)

Latuda… got me up and moving… helps with motivation

Seroquel helps quiet my head a bit so I can think in a straight line

Therapy helps me learn to block out the head circus noise…

The research doesn’t seem to show that this is helpful (though it seems there have been some conflicting results), but I’ve also heard from forum members that their doctors don’t recommend it… I think becuase its a stimulant and can increase risk of psychosis. Bottom line - talk to your doctor.

Some relevant research:

49 participants with chronic schizophrenia were enroled in a double-blind, placebo-controlled study across two sites and were randomised to either modafinil (200mg/day) or placebo. All participants engaged in a cognitive training program for 10 consecutive weekdays. The primary outcome measure was the performance on the trained tasks and secondary outcome measures included MATRICS cognitive battery, proxy measures of everyday functioning and symptom measures. 84% of the participants completed all study visits. Both groups showed significant improvement in the performance of the trained tasks suggesting potential for further learning. Modafinil did not induce differential enhancement on the performance of the trained tasks or any differential enhancement of the neuropsychological and functional measures compared to placebo.

Asenapine and lurasidone, two newer atypicals, have shown promise, as have agents that interact with the glutamate system. Another approach has been to add agents such as modafinil. More research is needed to consolidate the findings of these studies.

I got centrum multi-vitamins for cognitive improvement but I can’t swallow the pill it’s too big.

That’s true. High doses causes psychosis. I can only tolerate 100mg of modafinil. I think it helps little bit with fatigue and concentration.

Its funny nobody has mentioned sarcosine so far.

I read some people taking Alzheimer meds as an off-label to improve cognition like memantine and donepezil

Well you asked about “medicines” not vitamins and supplements.

Researchers are testing some of the medicines that also try for Alz. But none are very effective. The in-development drug encenicline is targeting both Sz and Alz.
