It was over the phone, no paperwork. It was some simple questions and a 13 minute recording reading me the privacy and reporting income information. The interview took about 30 minutes total, I liked it because it was a lot easier and faster then filling out a 5 or 6 page form. I think I get to keep my $23 a month benefits.
