I had an appointment with a psychiatrist to replace the one who moved away and the forms were crazy. “Are you in a sexual relationship” “are u sexually satisfied” “are u sexually dissatisfied”. Those were the only 3 multiple choice questions. Im asexual. I have no idea how to answer that. It also wanted me to list family members with their names. And other weird stuff. I just feel like these questionairs are too invasive. I have had my records lost on usb drives already like 4 times.
And when i go to the dermatologist they use an ipad to photograph my skin and face. Im sick of it. No privacy in medicine anymore. They act like they own u.
So long story short, i cancelled my appointment. ■■■■ it.

Asexual is covered right there.
It could be for finding out what your family medical history is. It could be for an emergency contact. These are legit questions.
Required for liability reasons. Some people will make claims against their dermatologist that are false and defamatory in nature. The picture can be used as protection against those claims. They are also used to show you before and after treatment. Also legit.
It wasnt. Its an online form where u can only select one answer. Nothing is that simple.
And dematologists never used to photograph. What did they do before. They wrote a description. Which i much prefer. I’m sick of having my naked body photographed. No one should have to consent to be photographed without clothes on just to make sure they’re moles are ok.
My mom went to the same derm for a mole right by her private parts. They took a photo of the whole area. It’s wrong.
As for the names. It wasnt for family history. There was nothing for that. They wanted the names of everyone u live with. And it wasnt under the family contact info either.
Def unusual. The whole thing is just inappropriate. Descriptions should be fine. Write the size. It’s in the record. What’s next. I wonder when u go to the psych ward now if instead of marking on a picture of a body if now they photograph u naked. If they don’t i wont be surprised when they start doing that.
They are not. I have unfortunately had to become familiar with liability law and legal precedent under the common law system in my own country, and there are different standards for different types of evidence. Video and photos are preferred to written descriptions. This isn’t going away no matter how much you dislike it.
On the topic. I hope someday it’s viewed as abuse to strip search patients on intake to psychiatric wards. I tried to refuse once. They told me if i didnt do it, they would tackle me down and strip me naked themselves. It feels so wrong.
I like forms, but I admit that I may be a little weird like that. I don’t like forms where I can’t write what I want. If I’m feeling goofy, or if the question is silly or seems irrelevant, I like coming up with strange answers. I figure it’s a nice change for the nurses who have to read the darned things.
I’m just explaining why they moved in the direction they did and that they are not going to move back. My own industry (insurance) was a nightmare when it was paper-based. Being paperless now makes us more efficient and we can pass on savings to customers (this actually happens, although I don’t think anyone believes that it does, LOL).
BTW, you should see the privacy regulations I deal with. Regular training and a $10,000 fine each for privacy breaches. No one messes with client privacy.

To be on the subject, paper filling is boring and lots of times I felt were there just to keep you busy, since you have to wait. But was a time when I used to have fun for short incomplet question like:
