Negative symptoms only is a version of sz called schizophrenia simplex, or simple schizophrenia. I think if it’s only positive symptoms it would just be psychosis NOS, but I’m not a doctor.
Paranoid schizophrenia has no prominent negative symptoms, mainly positive.
Simple schizophrenia has only negative.
And cognitive… i suffer from it … holy cow…!!!

I have no serious negative symptoms, and I’ve been diagnosed with undifferentiated schizophrenia. I do have some executive function challenges, but that’s because of a recent brain trauma.
Cognitive symptoms fall under negative symptoms.

I am not a clinician, so I am totally arm-chairing it here. I think what you’re describing is formally called ‘type II schizophrenia.’ Some people with schizophrenia stop having active symptoms but aren’t completely well - still have unusual thoughts, lack energy and motivation, etc.
The many proposed causes of this disease can be modified by many variations in the body. So each person with this disorder can have a number of presentations. For example, an infection may alter genetic action and cause an uneven presentation of clique symptoms. Also many medications drop positive dramatic troubles, but negative ones require more help.

To my knowledge, pharmaceutical companies spend millions of dollars a decade to bring most drugs to market. The limitation I see to this is that many drugs disappoint.
In other words, a series of different level studies must show the medicine is safe, that there’s a reasonable chance it will be helpful, and only then can a controlled study be done to demonstrate clear benefit. Many new touted “breakthrough” drugs prove terribly disappointing. This is the reality - there’s no magic way to find miracle cures, but I’ve never believed in dark conspiracies either.
Conversely, sometimes, a drug company can withhold research that is not beneficial to their product but not of a primary concern. However, that “minor” concern may later be important if scores of patients get an adverse event.
