Is there the possibility of success after clozapine fails

im on 900mg of clozapine for the past 9 months along with 300mg of Seroquel…but it has thus far NOT been a success for me…what do I do now

I’m assuming that you’ve been all of the other antipsychotics, so psychiatrists would say that you’re treatment-resistant.

Have you tried ECT and/or rTMS?

Yeah I was on clozapine 800mg for about a year. Did nothing for me. It is a myth that if clozapine fails you are screwed.

I went on a typical med called depixol which, along with quetiapine, has done good work where clozapine failed. I also added high dose Prozac and gabapentin for anxiety.

My advice is when clozapine fails treat the individual symptoms with multiple different classes of meds. It worked for me.

what is ECT or TMS…and yes ive been on a few anti p’s before this…by the way thanks for the input

Without doing any research, Electo-Convulsive Therapy (ECT) is a small amount of electricity administered to your entire brain. General anesthesia is necessary. Short-term memory is affected.

Rapid Tran-Cranial Magnetic Stimulation (rTMS) is magnetic pulses that can be administered to a specific part of the brain. General anesthesia not required.

I take Seroquel, and I’ve never had ECT and/or rTMS, so I’m not sure. That said, I’d prefer rTMS because a specific part of the brain can be stimulated.

@neveragain is currently having rTMS, so he would be able to best answer this question.

hi jimbob…when you say you were on clozapine 800mg for about a year…were you on that particular dose 800mg for that exact period of time 1year or was there a period of that time when you wee being titrated up…

what im saying is im on 900mg clozapine for the past 9 months but I have been on clozapine for a total of 1 year 7 months in total but 9 months at thehighest dose of 900mg

what im asking is how long specifically were you on the high dose of 800mg…and how long did it take to get you from the starting dose up to that level

also did your pdoc agree that you should come off it

Think I was titrated up to 400mg initially. On that for three months then titrated up to 800mg and was on that for 8 months. So one year in total.

Maybe you just need more time on clozapine at 900mg but 9 months seems like a long time.

Have you ever tried haldol (haloperidol)? It can be like rapid magic for some people.

Ps my doctor agreed I should try a different med. I wouldn’t recommend stopping meds without the input of a doctor. I have learned that through hard experience.

@karl don’t give up hope! If your pdoc doesn’t agree to the change, try another pdoc until you find one who does. Your body, your choices.

I know you want to get better and I know you’re doing the right things, things are going to improve for you just keep that good attitude you have and keep at it.
