The psychotic symptoms should only fluctuate the most, usually coming back to a baseline which is different for everybody. Negative & cognitive symptoms I think are more stable. Some people do believe that without antipsychotic treatment, further psychotic episodes will result in damage to the brain, worsening symptoms but this is quite debated. Schizophrenia is definitely not considered neuro degenerative. Many people have gotten better in some way either from antipsychotics, therapy, ECT etc.

If left untreated, there’s a bigger chance of it getting worse and worse.

After 10 years, of the people diagnosed with schizophrenia:

25% Completely Recover
25% Much Improved, relatively independent
25% Improved, but require extensive support network
15% Hospitalized, unimproved
10% Dead (Mostly Suicide)

After 30 years, of the people diagnosed with schizophrenia:

25% Completely Recover
35% Much Improved, relatively independent
15% Improved, but require extensive support network
10% Hospitalized, unimproved
15% Dead (Mostly Suicide)

My vision has improved tremendously. I had really bad Derealization for 5 years. Meds didn’t help. Time and therapy helped, I guess.

My paranoia, and delusions are worse. My negatives and cognitive have improved due to better meds. My anxiety has improved.
