I am on olanzapine 10 mg since the 20th of July.
I didn’t gain weight and I continue to exercise.
The intensity of intrusive thoughts and voices seems considerably reduced.
There are also positive signs regarding my sleep.
However, overall I feel bad and the cognition problem wasn’t solved and my functioning didn’t improve.
What do you think should I switch medications or dosage or should I stay on this and see what happens?

I have very hostile intrusive thoughts.
Before I went on medications I still had them and they were much more intense.
However I still have intrusive thoughts and I hate them.
Agree about certain antidepressants and intrusive thoughts. Ask your doc. There are a few that help, mostly the ones indicated for OCD. Also CBT may help. I think it is too early to switch meds.
I don’t notice any problem with your cognition in your posts. It sounds like the olanzapine is working at least a little. Maybe you could try some new med’s under the supervision of your doctor, but I wouldn’t be too aggressive with it.
On olanzapine I really started seeing some of my humanity returning after a number of months.
As for weight gain I’m sure it will creep up on you. Very few people take this drug and are able to avoid it.
Olanzapine is supposedly good for working memory, which is often the most impaired congitive domain in sz.
Remember, olanzapine is very similar to clozapine, in fact it was actually created to be as similar as possible without life threatening side effects.
Clozapine has a reputation for being something of a miracle drug for those who don’t respond to other harsher dopamine blockers.
It isn’t far fetched to say Olanzapine might be your miracle drug.
Remember, Rexulti is there too, and it is procognitive. I can testify to that. A combination of Rexulti and Seroquel XR had me earning straight a’s at community college. I felt so sharp and motivated.
Consider adding on Trintellix (Vortioxetine). It really helped me with every aspect of my cognition. In fact, I think some of that benefit might have been permanent.
Anna, I think you would find an antidepressant helpful for your intrusive thoughts, but ssri’s tend to cause sexual problems, I hope you are able to find a different antidepressant that you are happy with.
I have been on antidepressants for 12 years and every time I come off them my obsessive thoughts come back.

In the more than 3 weeks I am on it I actually lost weight!
A lot of people don’t gain weight on olanzapine it’s just scaremongering!

My cognitive impairment is very severe. All domains.
I had some impairment already in prodromal phase.
May be true but it’s not scaremongering. They’re required to list side-affects of medications if it’s happened to people, and people have gained weight a lot on Zyprexa (I did) and it happens. I’m glad it hasn’t happened to you yet and hope it continues that you don’t gain weight on zyprexa!
To give a med time for things like haze/fog to lessen you are talking 6 months at least which is what the psychiatrist told me last time I changed med. It takes 6 months for the changes to become normal to oneself and you become adjusted to it.
If you are on meds you have to face the fact that it is not all positive and that there are some bad points to taking a strong mind and physically altering drug. A few weeks effect is nothing. Give it time.
You need to give yourself time to acclimatize to the current meds. It can take weeks or months in some cases. We don’t get ill all at once, we don’t get better all at once either. I know you’re frustrated that everything isn’t where you want it to be yet, but the improvement we can see in you is incredible already. Please keep doing what you’re doing!
Hello @labratmat sometimes it takes time for meds to work.
However, I already feel a change compared to off meds.
Further, many people gain weight already in the first weeks of treatment with olanzapine, so that I haven’t gained weight is a good omen, although it doesn’t guarantee that it will continue this way.
