Participant Reported Insights from EMERGENT-5 In-Trial Interviews (Poster 195 and Poster 196)
In EMERGENT-5, a qualitative interview-based survey was administered to participants to characterize the self-reported patient experience and perceived changes in Quality of Life (QoL) when taking COBENFY. In the qualitative sub study, interviews were conducted at six weeks (T1; N=70) and six months (T2; N=47) after initiating treatment with COBENFY.
At study entry, most participants reported experiencing negative QoL impacts in the four assessed domains, including physical, social, emotional, and role functioning. Most participants described improvements in their QoL in at least one domain within six weeks of COBENFY treatment (86% improved, 13% unchanged, and 1% worsened).
At six weeks, most participants described experiencing a high level of satisfaction with COBENFY as compared to their prior antipsychotic treatment, which was sustained for up to six months of treatment. Participants also perceived improvements in symptoms of schizophrenia and QoL while reporting little burden associated with treatment as important contributors to their satisfaction. At six months, nearly all participants (93%) indicated that they would recommend COBENFY to a friend of family member, and 78% reported that they would continue COBENFY after the trial if given the option.
It seems that the medicine is also good for negative symptoms, which is why it is so expensive. Otherwise it would make no sense for the medicine to cost $1,850.

Cobenfy also significantly outperformed brexpiprazole in PANSS score improvements from baseline (mean difference [MD], -2.08; 95% CrI: -3.82 to -0.35). Cobenfy demonstrated comparable efficacy to the remaining atypical antipsychotics for the change from baseline PANSS scores.
Further, Cobenfy was associated with significant improvements in CGI-S scores relative to aripiprazole (MD, -0.33; 95% CrI, -0.57 to -0.11), cariprazine (MD, -0.38; 95% CrI, -0.62 to -0.15), and olanzapine (MD, -0.28; 95% CrI, -0.52 to -0.03), and similar efficacy relative to lumateperone, quetiapine, and olanzapine.
The investigators noted that Cobenfy demonstrated significantly lower odds of clinically meaningful weight gain vs all comparators except clozapine. However, Cobenfy had significantly higher odds for discontinuation across all causes vs all comparators (except cariprazine with a numerically unfavorable result).
“Xanomeline/trospium compares favorably to comparator atypical antipsychotics, the investigators concluded. “These findings support the compound as the first in a new class of medications to treat [schizophrenia] based on muscarinic receptor agonism, without any direct dopamine D2 receptor-blocking activity.”
Hmmm… Sounds like it will be good for some people and not for others.

I am posting this testimony again because he updated his post. He is losing a lot of weight, has no sexual problems, and sleeps well.
@Corian is on it. They say that it doesn’t appear to be helping.
I think in his case it is a question of efficacy. He also takes it in conjunction with other antipsychotics, not alone.

Im starting clozaril tonight and cobenfy is supposed to take a few weeks to know. Im two weeks and only notice heartburn.
I have a lot of delusions of control, voices and command voices. I just think i need a strong one but i duhnno we will see what works out
