" Has difficulties with perception"

This was written by a care co ordinator back when I had one . I have always wondered how this connects to psychosis/psychotic like thinking. Apparently hallucinations are ‘difficulties with perception’ but I think it can also mean a tendency to misinterpret events or put a spin on things that are not rational.
Is psychosis merely an absence of reality or is it also seen as a faulty/irrational interpretation of real events?

This topic is interesting but I also in need for answer.

I feel like this also refers to being out of touch with reality, which could be a general term for a lot of things, like hallucinations and delusions or even catatonic symptoms where you zone out or blank out for hours. But it could also refer to how you perceive events in your life. Like someone says one thing quite plainly and you come across a whole different meaning to it. But I have a feeling it mostly means some type of visual or thought distortion.
