Bruce says it at some of his live concerts. As far as I can gather, he borrowed the line from a famous American poet who lived in the twenties I believe. The poet was speaking about America, and he was talking about the shunned people in American society at the time. The poor, the minorities, the sickly, etc.
Springsteen, uses the line to mean the working class and the defranchised in America, upon who he bases his songs and his image. There’s a better explanation then this, but this is basically it.

It kind of cheapens the spirit of competition.

yeah, for the disabled it’s good though, like Special Olympics, sled hockey, volleyball, or other sports,

just that you tried your best, and my son loves it. My daughter got Hustler trophy in basketball camp,
it should be a surprise that you’ve gotten it, or maybe didn’t know it existed.
